Lets get going! If you are booking a PADI dive course if you haven’t already, can you please click read this PADI Medical Form  You must be able to answer NO to all 10 questions in section #1. IF you answer YES, you may still be able to scuba dive, however, you are required to take this form to a doctor and get it signed off as safe to dive before we can commence any water training activity. You will be required sign this form when your course begins.
